Services available

  • The Initial Comprehensive Assessment is a two to three hour in-person visit, interview and assessment of the older adult by the professional geriatric care manager evaluating physical, functional, environmental, financial, legal, psychological and social strengths, weaknesses, and concerns. It includes the development of an individualized plan of care based on the assessment findings and a care conference with the client and others as requested by the client to discuss the plan of care and recommendations/ options for care. All of this is provided for a one-time fee payable in advance. 
  • Ongoing monitoring and care coordination:  Many families rely on the geriatric care manager to provide guidance and assistance with ongoing care. The care manager will implement, coordinate, and monitor all, or part of, the plan of care. Services will be monitored to ensure that the original plan meets the older adult's needs, services are delivered, and modifications are made as the older adult's status changes. The care manager will coordinate care and keep lines of communication open with all involved parties.  This may be especially helpful when there are multiple health care providers.  Ongoing monitoring and care coordination is provided based on an hourly rate with an average of 2 to 4 hours spent each month.  Services are billed monthly.
  • The intensive assessment is an additional in-depth assessment provided by a recommended health care professional to address identified needs in more detail.  An example of this would be an in-depth home safety evaluation provided by a licensed therapist to identify specific environmental safety issues and a detailed plan to address the identified issues. Another example may be an individual counseling visit by a licensed social worker to address identified mental health issues. Intensive assessments would be provided only if recommended by the professional geriatric care manager and approved, in advance, by the client and/or family. Intensive assessments are billed at an hourly rate.
  • Technology, such as a personal emergency response system or automated medication administration system may be recommended. The geriatric care manager will help identify and arrange for these services if approved by the client. Fees vary based on the type of technology selected and will be billed monthly.
